Sejatinya, lagu "Not Like Us" menjadi diss track keempat dari Kendrick untuk Drake yang tengah berseteru. Liriknya juga secara terang-terangan menyebut nama Drake sebanyak dua kali.
Lagu ini melanjutkan tema yang pertama kali diperkenalkan dalam "Meet the Grahams", yaitu soal tuduhan pedofilia dan pelanggaran seksual yang dilakukan Drake. Bahkan, sampul lagunya juga menampilkan pemandangan rumah besar Drake di Park Lane Circle di Toronto, Kanada.
