Angka US$4,25 juta pada Lenmeldy atau US$3,5 juta pada Hemgenix merupakan harga daftar, bukan tagihan yang otomatis dibebankan langsung kepada pasien.
Produsen dan perusahaan asuransi dapat membuat diskon, rabat, hingga outcomes-based agreements—misalnya sebagian uang dikembalikan apabila terapi tidak menghasilkan outcome tertentu. Hemgenix, Lenmeldy, Zynteglo, dan beberapa terapi lainnya telah menggunakan atau menawarkan model semacam ini.
Biaya total perawatan bahkan bisa lebih tinggi dari harga obat. Pada terapi sel punca, pasien dapat membutuhkan kemoterapi mieloablatif, rawat inap, pengambilan dan pemrosesan sel, transfusi, hingga pemantauan bertahun-tahun.
Ada pula perubahan pasar yang membuat peringkat terus bergerak. Roctavian, terapi gen hemofilia A yang sebelumnya berharga sekitar US$2,9 juta dan kerap masuk daftar obat termahal, tidak dimasukkan di sini karena BioMarin mengumumkan penarikannya dari pasar pada 23 Februari 2026.
Beberapa terapi baru seperti Kresladi dan Waskyra juga belum dapat diperingkat secara pasti karena harga daftar publiknya belum tersedia saat artikel ini ditulis.
